ABSTRACT

OBJECTIVE To observe the effects of low-dose esketamine on analgesia and inflammatory factors after thoracoscopic surgery. METHODS Totally 120 patients who underwent thoracoscopic lobectomy in our hospital from October 2021 to March 2022 were selected and randomly divided into low-dose group (group A), conventional-dose group (group B), normal saline group (group C) by using the random number table method, with 40 patients in each group. All the patients were anesthetized with traditional general intravenous anesthesia, group A and B were anesthetized with low dose or normal dose (0.2 or 0.5 mg/kg) of Esketamine hydrochloride injection, and group C was given normal saline intravenously. The visual analog scale (VAS) score 0, 6, 24, 48 h after operation and the consumption of sufentanil 24 h after operation were compared among 3 groups. The levels of white blood cell count (WBC), neutrophil percentage, C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin-1 (IL-1) and IL-6 were compared 30 min before and 24 h after surgery. The Pittsburgh sleep quality index (PSQI) score and the Beck depression inventory (BDI) score were compared before operation and 0, 1, 2 d after operation, and the occurrence of adverse reaction was also recorded. RESULTS At 0, 6 and 24 h after operation, VAS scores and the consumption of sufentanil within 24 h after operation in group A and B were significantly lower than group C; VAS score 6 h after operation in group A was significantly lower than that in group B (P<0.05). The levels of WBC, neutrophil percentage, CRP, TNF-α, IL-1 and IL-6 in the 3 groups 24 h after surgery were significantly increased, compared with 30 min before surgery; the levels of above indexes in group A and B were significantly lower than those in group C (P<0.05). PSQI score and BDI score 0, 1 and 2 days after operation in group A and B were significantly lower than those in group C, while BDI score 0 day after operation in group A was significantly lower than that in group B (P<0.05). The total incidence of adverse reaction in group A, B and C were 5.0%, 10.0% and 17.5%, without statistical significance (P>0.05). CONCLUSIONS The low-dose esketamine has significant analgesic effect after thoracoscopic surgery, can reduce the levels of inflammatory factors after surgery and improve sleep quality and depression, with good safety.

ABSTRACT

Objective To develop the clinical application of tandem mass spectrometry in earlystage diagnosis of patients with growth retardation. Methods Tandem mass spectrometry was used to quantitatively detect blood amino acids and acyl carnitine in 155 cases of children with growth retardation. The tandem mass spectrometry results, clinical symptoms, and treatment tracking results were analyzed. Results Ten patients with inborn metabolic absence of amino acids and acyl carnitine were confirmed including four cases of methylmalonic acidemia, one case of propionic acidemia, one case of medium-chain acyl-coenzyme A dehydrogenase deficiency, one case of ornithine transcarbamylase deficiency, and three cases of phenylketonuria. Conclusions Tandem mass spectrometry was helpful for some patients in etiologic diagnosis and therapeutic effect assessment of cerebral developmental retardation.

ABSTRACT

AIM:To clarify the effects of high fat/cholesterol diet on lipid metabolism and atherogenesis in treble genes mutant mice.METHODS:ApoE-/-/LDLR-/-/Leprdb/db mice were generated by cross apolipoprotein E,lower density lipoprotein receptor gene knockout mice with leptin receptor gene spontaneous point mutants. The mice were fed with high fat/cholesterol diet from 22-day-old.The total plasma cholesterol(TC),triglyceride(TG)and glucose levels were measured and pathological changes of aorta intima and liver were analyzed.RESULTS:A significant elevated TC,TG and glucose levels in plasma with progress of time in young treble gene mutant mice were observed, which were higher than that in ApoE-/-/LDLR-/-and Leprdb/db mutants. At time of only 2 weeks after fed with high fat/cholesterol diet,TC and TG levels reached(106.75?3.40)mmol/L,(9.12?1.35)mmol/L,respectively in treble gene mutant mice,4.33-and 2.36-fold higher than those in treble genes mutants fed with normal chow diet. The levels were continuously increased until final experimental point.Intima of the aorta appeared with various injuries such as edema, desquamation of the endothelial cells, foam cell formation, rupture of IEL in local regions of root and arch areas of aorta at 2 weeks after fed with high fat/cholesterol diet.Microscopic pathological complex of significant local intima incrassation and fatty change of the liver were observed in the mutants that fed with high fat/cholesterol diet for 8 weeks.Injuries of aorta were severe than normal dietetic control group.CONCLUSION:High fat/cholesterol diet as a key dietary factor is significant aggravated lipid metabolism abnormity, promotes early damage of aorta and process of atherogenesis in the treble genes mutants.

ABSTRACT

AIM: To analyze the nucleotide and putative amino acid sequences of PIA genes isolated from N. gonorrhoeae and to construct the prokaryotic expression system of PIA gene.METHODS: The entire PIA genes from 9 strains of N. gonorrhoeae were amplified by using high fidelity PCR. The target amplification fragments were sequenced after T-A cloning. Homology comparison of the nucleotide and putative amino acid sequences of PIA genes from the isolates with the reported sequences in GenBank was then performed. A prokaryotic expression system of PIA gene was constructed. Different dosages of IPTG were applied to induce the expression of the target recombinant protein (rPIA) and 10% SDS-PAGE plus Bio-Rad Agarose Image Analysor was used to determine the expression level of rPIA. rPIA was extracted using Ni-NTA affinity chromatography and the purified effect was detected by SDS-PAGE.RESULTS: In comparison with the reported PIA gene sequences (GenBank No: L19962), the homologies of nucleotide and putative amino acid sequences of PIA genes from the isolates were 99.6%-100% and 99.1%-100%, respectively, which indicated that all the isolates were belonging to serovars IA6. Output of rPIA was as high as 50.1% of the total bacterial proteins. The purified rPIA only showed a single target protein fragment in gel.CONCLUSION: Serovar IA6 is dominant in the local N. gonorrhoeae isolates and sequences of the encoding gene are relatively conserved. The constructed prokaryotic expression system is able to express rPIA with high efficiency, which may lay a foundation for further development of serological detection kit and vaccine of N. gonorrhoeae.
